Chronic elbow pain describes persistent or frequently recurring pain in the elbow region lasting beyond the expected recovery period for acute injury — typically three months or longer. The most common causes are lateral epicondylitis ('tennis elbow' — extensor tendinopathy of the common extensor origin at the lateral epicondyle), medial epicondylitis ('golfer's elbow' — flexor tendinopathy at the medial epicondyle), and cubital tunnel syndrome (ulnar nerve compression producing medial elbow pain with numbness in the fourth and fifth fingers). Less commonly, olecranon bursitis, arthritis of the elbow joint, and referred pain from the cervical spine contribute. Chronic elbow pain significantly affects grip-dependent daily activities and occupational function. Eccentric loading programmes have the strongest evidence for tendinopathic elbow pain.

Chronic elbow pain is well-studied and often manageable
Most chronic elbow pain has identifiable mechanical or nerve-related causes with established management pathways. Eccentric loading exercises, ergonomic changes, and structured rehabilitation have the strongest support for tendinopathic presentations.
Seek prompt assessment if you experience sudden loss of elbow mobility, joint swelling with redness and heat, symptoms following a fall or trauma, or progressive weakness and loss of function. These presentations may indicate fracture, infection, inflammatory arthritis, or nerve injury that requires timely professional evaluation.
Eccentric and isometric loading programmes are well-supported for lateral and medial epicondylitis. Ergonomic adjustments and activity modification also have consistent backing. Evidence for other approaches — including manual therapy and acupuncture — is more limited and generally considered adjunctive rather than primary.
