Tender points refer to 18 specific bilateral anatomical locations that, when palpated with approximately 4kg of pressure (sufficient to blanch the thumbnail), produce significant pain in individuals with fibromyalgia. The 1990 ACR criteria required 11 or more of these points to be positive for fibromyalgia diagnosis. The 2010 and 2016 revised ACR criteria moved away from tender point counting toward a symptom-based approach incorporating widespread pain index (WPI) and symptom severity scale (SSS), reflecting the understanding that fibromyalgia is primarily a disorder of central sensitisation rather than peripheral tissue pathology. The classic tender point locations include the suboccipital muscle insertions, lower cervical spine, trapezius muscle, supraspinatus muscle, second rib, lateral epicondyle, gluteal region, greater trochanter, and medial knee. Tenderness at these sites indicates abnormal pain processing — hypersensitivity to stimuli that would be non-painful in unaffected individuals — rather than structural tissue damage at those locations.

Tender points reflect central pain sensitisation, not local tissue damage
Tender points are well-studied markers of abnormal pain processing in fibromyalgia. Multimodal approaches — combining movement, psychological support, and targeted therapies — have the strongest evidence for reducing tenderness and improving function.
Widespread pain accompanied by fever, unexplained weight loss, or night sweats warrants medical assessment to rule out systemic causes. Asymmetric or single-region tenderness with neurological symptoms needs structural evaluation. Rapidly worsening pain and tenderness should be assessed to exclude inflammatory arthropathy or infection.
Aerobic exercise has strong evidence for reducing pain, fatigue, and tender point count. Cognitive behavioural therapy has strong evidence for improving function and reducing pain catastrophising. Mindfulness-based stress reduction shows moderate to strong evidence. Pharmacological options including duloxetine, pregabalin, and amitriptyline have established evidence for symptom management.

Revised 2010 and 2016 criteria shifted toward symptom-based scoring, reflecting evidence that fibromyalgia involves disordered central pain processing. Tenderness at these sites indicates hypersensitivity to stimuli rather than local structural damage.
Traditional Chinese Medicine may interpret widespread tenderness through patterns such as Liver Qi stagnation or Kidney deficiency, using acupuncture, Gua Sha, and Tui Na. Acupuncture has moderate evidence for pain and tender point reduction. Ayurveda relates similar presentations to Vata imbalance, using gentle oleation and dietary approaches. These frameworks are not substitutes for professional assessment.
High-intensity exercise during an active fibromyalgia flare may worsen symptoms — graded pacing is essential and supported by evidence. Deep pressure massage directly over active tender points can temporarily increase pain and should be approached cautiously. Low-dose naltrexone shows emerging evidence but should only be considered under qualified medical supervision.
Evidence supports combining graded aerobic exercise, psychological therapies such as CBT, and mindfulness practices as a foundation. Complementary approaches including acupuncture and somatic therapies may add value alongside this foundation. Working with a qualified practitioner to coordinate care across modalities is advisable, particularly where symptoms are complex or longstanding.
