In myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS), extreme fatigue with post-exertional malaise (worsening of all symptoms after any activity) is the defining feature and must be managed through careful energy management. In autoimmune conditions (lupus, rheumatoid arthritis, multiple sclerosis), extreme fatigue reflects the inflammatory burden on the body.
