What is ME/CFS and why is it so often misunderstood?

Myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) is a serious, complex, multi-system illness characterised by profound fatigue, post-exertional malaise, cognitive dysfunction, orthostatic intolerance, and unrefreshing sleep. It is frequently misunderstood because it has no diagnostic biomarker, symptoms overlap with other conditions, and it was historically (incorrectly) attributed to deconditioning or psychological factors alone. ME/CFS is now recognised as a genuine neuroimmune condition with measurable abnormalities in immune function, mitochondrial function, and autonomic regulation.

Why is "graded exercise therapy" controversial in ME/CFS?

Graded exercise therapy (GET) was previously recommended for ME/CFS based on a deconditioning model of the illness. However, subsequent patient surveys and reanalysis of the PACE trial have shown that GET worsens outcomes for many ME/CFS patients through post-exertional malaise — a specific physiological response in ME/CFS where exertion triggers relapse lasting days to weeks. The model of deconditioning does not adequately explain ME/CFS. Current NICE guidelines (2021) recommend against GET and instead advocate energy management and pacing approaches.

What is pacing and why is it central to ME/CFS management?

Pacing involves staying within one's "energy envelope" — the level of activity that can be sustained without triggering post-exertional malaise. Rather than pushing through fatigue, pacing means monitoring energy expenditure, resting proactively before depletion, and gradually finding stable activity levels that do not exceed the body's available capacity. Heart rate monitoring (keeping below a symptom threshold, typically 110 bpm or less depending on the individual) is used by some to guide pacing objectively. The goal is preventing boom-bust cycles that prolong and worsen illness.

What is the current evidence for treatment of ME/CFS?

There are currently no treatments with strong evidence for fundamentally altering ME/CFS. Low-dose naltrexone, omega-3 fatty acids, and CoQ10 have emerging evidence for symptom reduction in some patients. Symptomatic management of sleep, pain, and orthostatic intolerance is important. Nutritional assessment and correction of deficiencies (particularly B12, D, and magnesium) is appropriate. Supportive psychological therapy (not GET or CBT aimed at altering illness beliefs) can support quality of life and coping. Research into ME/CFS has accelerated significantly since COVID, given the overlap with long COVID.
