Muscle stiffness is common and usually mechanical in origin, with strong evidence supporting movement, heat, and hydration. Distinguishing everyday stiffness from pathological causes is important — some underlying conditions require professional assessment.
Seek professional assessment if stiffness follows trauma or a fall, is accompanied by joint swelling, redness, or heat, or involves sudden loss of mobility. Progressive weakness, loss of function, or stiffness that worsens over weeks without clear cause also warrants evaluation. These presentations may indicate conditions that need clinical investigation rather than self-care.
Regular stretching and movement have strong evidence for reducing musculoskeletal stiffness, particularly after inactivity or exertion. Heat therapy is well-supported for improving tissue extensibility and reducing perceived tightness. Where stiffness has an underlying cause — such as thyroid dysfunction — addressing that cause tends to resolve symptoms more effectively than symptomatic approaches alone.
Everyday stiffness from exertion, cold, or prolonged sitting is physiological and generally self-limiting. Pathological causes — including hypothyroidism, inflammatory muscle conditions, statin use, and fibromyalgia — produce stiffness through different mechanisms and require professional assessment. Persistent or unexplained stiffness, especially with fatigue or systemic symptoms, should not be attributed to lifestyle factors without evaluation.
Physiotherapy, massage, yoga, and structured movement programmes are widely used for musculoskeletal stiffness and have reasonable evidence bases. Complementary approaches such as acupuncture and myofascial release are used by some practitioners, with limited to moderate evidence.
